ACH - Prospect


I (we) hereby authorize Lee’s Tax City, LLC /dba: 1040TaxBiz to initiate ACH transactions to the banking institution and account named as indicated below for the purpose of tax preparation revenue transactions. This authorization is to remain in effect until 1040TaxBiz receives written notification of termination or change of account information.

The credit card will be charged for the following circumstances:

  1. If the client does not achieve a minimum of (5) BANK PRODUCTS or (10) EFiles between January 1, 2025, and April 15, 2025 (TAX DAY). Failure to meet the minimum requirements for Refund Transfers or EFiles will result in the client being liable to pay $299 at the conclusion of the tax season.
  2. If the client cancels after 14 days of receiving login information, 1040 TaxBiz will impose a $299 charge for the initial setup and software licensing provided.
  3. In the event of a "Return ACH" when 1040 TaxBiz initiates ACH debit for weekly EFiles fees, the credit card will serve as a backup method for payment.
